- Please complete all the information and mail or fax it back to us.
- Once we have all the information, our team will call you when it is time to schedule the appointment.
Medicaid Transportation
- If your child is a member of Iowa Medicaid, you might qualify for non-emergency medical transportation to your appointment at the Center for Disabilities and Development. This includes reimbursement for gas, food, and lodging. Please find more information here: Medical Transportation.
