Completely fill out all parent forms in this packet. 

Make sure to write your child’s name and date of birth on every form. 

If forms are not completely filled out, they  will be sent back to you for completion.

  • Follow the directions on the forms. Please do not leave any questions blank.
    If the question does not apply to your child or you have not witnessed your child 
    doing this behavior, please mark the question as zero.

  • Only mark ONE answer for each multiple-choice question.

  • Give Teacher forms to ONE teacher to fill outIf your child has more than one teacher, we suggest giving forms to Math, Science, Language Arts, or Special Education teachers. The teacher should send the forms back to our office via fax or email.

  • If your child is homeschooled, please fill out the teacher form yourself, focusing on behaviors seen during school hours.­

  • Once all forms are received, they will be entered into your child’s medical chart. Please allow time for our team to review and determine the appropriate provider for your child. Our scheduling team will call you when the appointment is ready to be scheduled.

Returning the Packet to CDD

A postage-paid envelope is included in this packet to return your paperwork via US Mail. You can also email a .pdf file or fax the paperwork back to our office. This is often a much quicker option. Our fax and email information are located at the bottom of the webpage.

How Do I Get Teacher Forms Filled Out in the Summer?

We understand that during the summer vacation, teachers are not available to complete paperwork. Returning the forms in the fall completed either by the teacher from the previous school year or in the fall of the current school year will still get your child scheduled in a timely manner. Most of our clinics schedule out 6 months or more, therefore all children requiring school paperwork are in the same position, so your child’s appointment should not be delayed.

What Should I Do if I Misplace the Packet of Paperwork?

We understand that during the summer vacation, teachers are not available to complete paperwork. If You Need to Cancel or Reschedule Your Appointment

Please call our schedulers so they can fill your appointment time with another patient that is on the waitlist. Dial 877-686-0031 and press option 1 for appointments, then option 2 for CDD appointments.

Where is the Center for Disabilities and Development Located?

CDD is in a 3-story brick building located at 100 Hawkins Drive in Iowa City. We are NOT located in the main hospital or the children’s hospital. We are between the Dental College building and the Main Hospital.

Parking - We Have FREE Parking in the CDD Lot

Park in any available space in the front and to the side of our building. You do NOT need to register your car with the ParkMobile app. Please note your license plate number and give it to the clerk at check-in.  This will prevent you from getting a parking ticket.  If you park in one of the ramps, a parking pass can be printed for you at checkout.

Food Options

We do not have a cafeteria at the CDD but there are several cafeterias at the main hospital. Stop by our front desk and our registration staff will be glad to give you directions!   We do have several vending machines with snacks, small meal options, and drinks. Our Vending Machines take Cash, Credit/Debit Cards and Apple Pay.  We do not have an ATM machine in our building, the closest ATM machine is at the main hospital. 

What Should I Bring to My Appointment?

We recommend you bring a portable toy or activity for your child. If your child uses any equipment (AAC Device, Wheelchair, walker, etc.) we ask that you bring that to the appointment.

What Are the Expectations For Parents/Guardians/Caregivers During The Appointment?

We require that a caregiver be present for each appointment. If a parent/legal guardian is unable to attend, another trusted adult may bring the patient to the appointment with a signed consent form. Please notify our staff at 319-356-1346 if you need a consent form for someone else to bring the patient.

While You Are Waiting, Please Be Sure To

  • Visit our playground (in the courtyard outside the 3rd floor entrance) when weather permits.
  • Visit our Disability Resource Library (1st floor entrance) where you can pick out a free book (for first time patients)
  • Ask about our building-wide scavenger hunt at the registration desk. Once completed you can pick up a prize at the Disability Resource Library or the Registration Desk.
