As you prepare for your visit at the Center for Disabilities and Development, here is some important information to help you plan.  

  • In most cases, you will receive a packet in the mail.
    • Please complete all the information and mail or fax it back to us.
    • Once we have all the information, our team will call you when it is time to schedule the appointment.

Medicaid Transportation

  • If your child is a member of Iowa Medicaid, you might qualify for non-emergency medical transportation to your appointment at the Center for Disabilities and Development. Accommodations   

DRL 

The Center for Disabilities and Development has the Disability Resource Library.
