What’s a Transition Plan—and Why Does It Matter?
A transition plan helps students with disabilities prepare for big changes—like starting a new school year, moving to a new grade, or building life skills for adulthood. Whether your child is entering kindergarten or high school, having a clear plan in place can reduce stress and build confidence. Here’s what to include in a transition plan:
- Goals for the upcoming school year
- A review of supports and accommodations
- Routines and expectations—both at school and home
- Opportunities for independence and self-advocacy
- Communication strategies between families and school staff
Start small. Talk with your child about what to expect, walk through the school building together, and build in visual schedules or social stories as needed. Transition isn’t just about getting from one place to another—it’s about helping students feel safe, supported, and seen.
